The closer to the nerve the cavity is, the more likely you will need a root canal. From that picture, it looks like you will need a root canal.answers 2:filling should be enough!answers 3:If I was the dentist I would remove all of the decay and the assess the status of the nerve. If the nerve is still alive, even if the decay went into the nerve , I would treat the pulp and see if the damage will heal and the tooth will be okay. You can always ! do a root canal in the future if the tooth would die. Remember a root canal on a molar has a chance of success around 90 %. Good luck.
